Purpose: A program must provide a newborn visit with each mother and baby to offer support and identify family needs.

When: A program must schedule the newborn visit within two weeks after the infant’s birth. 

  1. Child/Family Development Specialist
    Contacts Health Specialist, their Area Manager, the Health Consultant, and the Mental Health Consultant following the birth of the enrolled mother’s baby. Schedules home visit before infant is two weeks old.
  2. Health Consultant & Child/Family
    Provides a Post-Partum Assessment/Newborn Development Specialist Home Visit.
    The Health Consultant completes the Post-Partum Assessment/Newborn Home Visit form.
    The Health Consultant makes a copy for her use and gives the original to Health Specialist.
  3. Health Specialist
    Reviews the form and enters a Health Consultant event in ChildPlus/Health for Post-Partum/Newborn Home Visit. Sends the form to the Child/Family Development Specialist to file in Site File.
  4. Child/Family Development Specialist
    Reviews Post-Partum Assessment/Newborn Home Visit information on form and in ChildPlus and completes follow-up stated on form.
