  • This form will be completed after each checkpoint within 3 weeks and will be done after each quarterly assessment.
  • List what DRDP reports you used to analyze the data and you may also use other forms such as the child’s growth between milestones or ASQ, etc.
  • List each child and age that is on your caseload or in the classroom.
  • Use the Head Start Early Learning Frameworks to guide you in what is below typical development.
  • In the other information column document if the child is enrolled less than 90 days and what the enrollment date was. You may also use this column for any attendance issues.
  • Review your observations and look at areas that you might want to gain more information in the next quarter. Do you have parent observations? Are there materials that you can plan for that may give you an opportunity to see the child using the skill that you are trying to observe?
  • Look for patterns or trends that you might notice with each child or as a group.
  • Then think of ideas and ways to support parents in gathering observations, implementing activities especially for children who are below typical development but also for those parents that may need more support in implementing activities at home.
  • After reviewing all this information then you will think about how this data will guide your teaching and learning.
  • Review with Area Manager and document thoughts and any other things that may need to be implemented.
  • Area Manager will upload to shared google drive and this data will be used to guide program trainings, coaching and next steps.
